No matter what I do, PSPad won't save settings, such as:
Settings>Program Settings...>Editor Part 2>Real Tabs

I've tried it while logged in as both standard User and Administrator on Windows. Before reinstalling the OS and then PSPad, it worked fine.

Windows 7, SP1
PSPad v 4.5.7 build 2450

Please ensure that you don't install PSPad as portable. No application has rights to write into Program files folder.

You have 2 ways how to fix it:
1. Run PSPad installer again and uncheck option "Portable application" during setup.
2. Run PSPad as administrator (use right mouse) to elevate PSpad rights, go to Program settings/System and check Multiuser environment

Your previous setting will be preserverd.

Is it possible to export settings before the reinstall?

When you install new PSPad version and you will reinstall it, not uninstall and install new, your settings will be preserved.

You can export settings anytime from PSPad. Open Program settings / Backup
There are 2 buttons for backup and restore settings.
