Jan,
Installed PSPad 64 using installer; I went into Settings > System Integration to turn off Hex. This removed the whole context menu for PSPad. Now I can't enable it. This was with either the dll. Using the registry seems to work.
Doing an uninstall and reinstall after removing PSPad settings in AppData didn't work.
If I run regsvr32 /n /i:everyone "C:\Program Files\PSPad Editor\pspshellx64.dll" I can get it to work.
Not sure If did something wrong or something is broken.

Hello

I tried:
regsvr32 -u "drive:path\pspshellx64.dll"
it sucessfully removed context menu items
regsvr32 "drive:path\pspshellx64.dll"
added context menu items back

Your case, when you register DLL for everyone must be called from elevated CMD (started as admin)
I tested it too, DLL was sucessfully loaded and menu items appeared.

To start PSPad from the context menu, you need at least once start PSPad. This write path to PSPad into registry and shell DLL use it.
It means shell DLL can be placed anywhere and it will start last used PSpad.
