I had the same problem - and found a solution! 
So for everyone like me who doesn't like to run dubious exe-files posted in the forum (which is down anyway), change the registry setting yourself like this:
HKEY_CLASSES_ROOT --> Applications --> PSPad.exe --> shell --> open --> Command
add "%1" to the end of the path:
Name: (Standard)
Type: REG_SZ
Value: "C:\Program Files\PSPad\PSPad.exe" "%1"

Had the same problem now in Windows 8. Just running PSPADShell_unreg.bat actually fixed the problem that it won't open the file.

Hi!
Is the patch-source still available? I got a very new Windows Server 8 64bit. Double click to a file does not open it! What was the solution of PSPad_patch? Why is it a secret???? Where can I download the source?

Hi!
I got it. It was only to .cfg-files. I used Program settings / System to register filetype .cfg. Now it works.

What are you talking about? There is no secret patch. Do you use the latest PSPad version?
Check the registry, as is mentioned here or run PSPad as administrator, in the program settings / System integration uncheck all integration, confirm and check back.
